Ann Eubank Dzuryak, 97, of Morris, IL passed away peacefully on Tuesday, May 3, 2016 at Park Pointe Senior Living in Morris, IL. She was born April 2, 1919 in Westfield, WI the daughter of the late Hiram J. and Cynthia Maude (Stewart) Eubank. She graduated from Westfield High School (Class of 1936), attended Marquette University in Milwaukee, WI and graduated from St. Mary's School of Nursing/Mayo Clinic in Rochester, MN (Class of 1940). She married Joseph J Dzuryak in 1944. She was employed for several years as an Industrial Nurse prior to retiring in1991 from Alumax Mill Products, Inc. in Morris, IL. She was a member of the First Presbyterian Church in Morris, IL, Order of the Eastern Star and Eubank-Ulrich-Zuelke VFW Post 8718 Auxiliary of Westfield, WI, the Ada Bliss Chapter DAR and a former volunteer of the Morris Community Hospital Auxiliary. Survivors include two sons Tom (Sheryl) Dzuryak of Morris, IL, J. John (Francine Rajski-Dzuryak) Dzuryak of River Forest, IL, grandchildren John-Franklin Dzuryak of Humble, TX, Kim (Jeremey) Phillips and their children Gage, Ayla and Quaid all of Morris, IL; Chris (Suzy) Brown and their children Morgan, Luke, Brady, Joe and Jack all of Morris, IL; a nephew Tom (Betsy) McIlwain of Phoenixville, PA. She was preceded in death by two daughters Joann Mary Dzuryak in 1969, Cynthia R. Dzuryak in 2010, two sisters Ruth McIlwain, Mary Larson and a brother Lt. Thomas Eubank. Visitation will be on Friday, May 6, 2016 from 10:00 AM until the funeral service at 11:00 AM at the First Baptist Church 1650 West Route #6 Morris, IL. Interment will take place at Evergreen Cemetery in Morris, IL. Memorials may be directed to Grundy Community Volunteer Hospice 518 West Illinois Ave Morris, IL 60450.
